El programa de salud de la Caja de Compensación Familiar de Sucre COMFASUCRE, en aras de garantizar la atención integral en salud de sus afiliados, se hace necesario la conformación de la red de prestadores para el año 2022, teniendo en cuenta condiciones de accesibilidad, oportunidad, continuidad, integralidad, resolutividad y calidad, de acuerdo a la oferta de servicios de salud y a las necesidades de los afiliados.
En ese orden, dentro del procedimiento de gestión contractual del programa de salud de Comfasucre, se le extiende la invitación a ofertar servicios a Instituciones Prestadoras de Salud, Empresas Sociales del Estado y demás proveedores de servicios de salud, que puedan cumplir a cabalidad su objeto social, con el fin de garantizar la pluralidad, transparencia y eficiencia de sus recursos logrando así mejores condiciones contractuales y de prestación de servicios que redunde en beneficio de sus afiliados.
En caso de encontrarse interesado (a) podrá hacer llegar la información a la oficina de archivo central o en la oficina de Administración de la Red.
Los documentos que deberá anexar son los siguientes:
1. Portafolio de servicios.
2. Diligenciamiento formato de servicios ofertados.
3. Diligenciamiento formato de medicamentos y suplementos ofertados.
4. Diligenciamiento formato de insumos médicos ( este formato se debe diligenciar de acuerdo al listado institucional de Comfasucre EPS INSUMOS_ODONTOLOGICOS_MEDICO QUIRURGICO_REACTIVO DIAGNOSTICO_REACTIVO INVITRO)
5. Diligenciamiento formato de material de osteosíntesis ofertados.
6. Documento que acredite la representación legal (vigente a la fecha)
7. Fotocopia de cédula de ciudadanía del representante legal
8. Fotocopia del Rut.
9. Certificado de habilitación
10. Certificado de antecedentes disciplinarios- no mayor a tres meses de la fecha de su expedición (Representante Legal – IPS)
11. Certificado de antecedentes fiscales – no mayor a tres meses de la fecha de su expedición (Representante Legal – IPS)
12. Certificado de antecedentes judiciales- no mayor a tres meses de la fecha de su expedición (Representante Legal – IPS)
13. Póliza de responsabilidad civil médica (vigente)
14. Soporte capacidad instalada disponible para Comfasucre.

16. Indicadores de calidad hasta la fecha: resolución 1552 de 2013, resolución 256 de 2016 y demás indicadores de oportunidad y de riesgo que aplique de acuerdo al servicio ofertado.
